Health Programs and Classes

The Living Well Self-Management Programs are evidence based seven-week workshops for adults with more than one health issue who live in the District of Columbia. Each workshop lasts for 2 ½ hours and happens once a week. The workshops are also held in easy to get to places in the city. Anyone who has a disease or health problem can come to the workshop and bring their friends or family. Workshops are taught by two trained leaders, one or both of whom are residents of the District of Columbia with a disease or health problem.

The DC Department of Health partners with community organizations to offer:

  • Chronic Disease Self-Management Program
  • Diabetes Self-Management Program
  • Diabetes Prevention Program 
  • Cancer: Thriving and Surviving

If you or someone you care for has an ongoing health condition such as diabetes, obesity, heart disease, high blood pressure, chronic pain, depression anxiety or cancer, the Living Well Workshops can help.

Most workshops are free.

Diabetes Self-Management Education & Training Programs (DSME/T) help people diagnosed with diabetes acquire knowledge and skill to navigate decisions and activities that improve health outcomes. Diabetes care is complex and involves many healthcare professionals however DSME is a resource used to address the challenges of ongoing self-management. DSME/T programs are reimbursed by Medicare and many insurance plans.

The National Diabetes Prevention Program—or National DPP—is a year-long, evidence-based program for people diagnosed with pre-diabetes. Participants who complete the program can reduce their risk of developing type 2 diabetes by 58%. Select here to take the ADA’s Prediabetes Risk Test.
