  • Addressing Social Determinants of Health to Achieve Health Equity

This session will include a presentation on the connection between health equity, health disparities, and social determinants of health (SDOH), and how to apply a systems-thinking and health equity informed lens to their efforts to mitigate health disparities and SDOH in their patient population. The presentation and discussion will explore Million Hearts Strategies to achieve health equity as they related to Grantee activities.

Kate Milone will discuss implementation challenges and solutions for Elaine Ellis Center of Health’s self-monitoring blood pressure (SMBP) program.
