  • Developing, Testing and Adapting New Workflows
    Facilitated Discussion – Grantee Spotlights

The Million Hearts project requires Grantees to integrate new (or adapted) workflows into the practice. This is a complex, multi-step process, including developing the new workflow, shifting staff responsibilities, training staff to implement the workflow, monitoring its use, adapting it if needed, and promoting its continued use. During this session, two grantees will share learnings and challenges related to specific workflows to support MH activities.

  • Jeff Weinfeld, MD, MBI, FAAFP, Million Hearts Project Director, Professor of Family Medicine, MedStar Georgetown Family Practice
    Dr. Weinfeld will discuss workflow development to support home blood pressure monitoring.
  • Ricardo Fernandez, MD, Chief Medical Officer, La Clinica del Pueblo
    Dr. Fernandez and the team from La Clinica will discuss workflow development to increase statin use for diabetes.
  • Facilitators: Mary Kate Brousseau, Kristina Ramos-Callan

 

  • Prescribe the Y – Referrals into YMCA Health programs

Ms. McCarron will provide an overview of the Y’s evidence-based health interventions, workflows to support referrals, and opportunities for MH Grantees.

  • Speakers: Kristy McCarron, MPH, Vice President, Community Health and Wellness, YMCA of Metropolitan Washington
  • Intended Audience: QI leads, Care coordinators, entire clinic care team
