    The session will include a brief update from the Chronic Disease Division’s data team to share the status of grantee progress with improving Million Hearts metrics and trends noted thus far.

    MHLC grantee Bread for the City will discuss their Produce Rx initiative and patient engagement strategies that go with it.

    Patient Engagement: Barriers and Opportunities

    Patient engagement is often subject to barriers and challenges that seem out of the clinic staff’s control. During this session, we will review common themes in patient engagement barriers and facilitators, as well as considerations for moving from patient engagement to patient empowerment in their care. Learning Session participants will also have an opportunity to share patient engagement challenges and strategies in small group discussions with other MHLC grantees and collaborative stakeholders.
