The focus of the meeting is to:
- Share DC Health’s expanded Million Hearts Learning Collaborative Action Plan and Goals
- Introduce DC Health’s Innovative Cardiovascular Disease Learning Collaborative Action Plan and Goals
- Ignite enthusiasm about heart disease and stroke
- Build relationships and community among participants
- Gain consensus and commit to action
Since 2013, the Million Hearts Learning Collaborative has engaged health system and quality improvement partners in a shared space with the goal of reducing heart disease and stroke incidence and mortality in the District. We recognize the need to close the chasm between clinical medicine and public health and aim to create a community of healthcare professionals, researchers, community partners and residents dedicated to preventing and managing heart disease and stroke through collaborative learning, knowledge sharing, evidence-based practices, and innovative solutions. We are diligently seeking participation in this learning collaborative from clinicians, nurses, CHWs, pharmacists, social service community-based staff, QI teams, and front-line staff at multiple clinic locations.
