July 2026: District-wide Heart Disease and Stroke Prevention & Innovative Heart Health Learning Collaboratives

This session featured presentations from DC Primary Care Association, Howard University Hospital, and Bread for the City on strategies to improve cardiovascular and chronic disease outcomes. Presenters shared lessons learned from implementing remote patient monitoring, continuous glucose monitoring, and workflow improvements to increase blood pressure screening and chronic disease management. The session highlighted implementation successes, challenges, health equity considerations, and practical approaches for improving patient outcomes.
