MCO Partnerships for Million Hearts Population: Opportunities with AmeriHealth Caritas
The session includes an overview of opportunities to partner with AmeriHealth Caritas in regard to improving care for patients with diabetes, hypertension and high cholesterol. Representatives from AmeriHealth Caritas provide a brief overview of services offered for this population and how practices can partner with the Managed Care Organization (MCO) for better patient care. Topics discussed include care coordination, community partners and programs, access to data, and value-based payment.
