The session will present a brief review of key principles and strategies to building successful team-based care. The session will also showcase an example of working with external partner, Sorogi, to augment your team to support Million Hearts chronic conditions. Dr. Ayuk will discuss Sorogi’s collaborative approach to building workflows and communications alongside health system teams and describe lessons learned and noted improvements thus far.
