In this presentation, Whitman-Walker Health will showcase how the team will utilize the care coordination and quality improvement tools of its new electronic health record - Epic - to bolster its Lean process improvement work on cardiovascular outcomes for its patients, including Black and African American Men.

The Family and Medical Counseling Service team will review two PDSAs:
1. Shortening their education series, which boosted enrollment and reduced
attrition, and,
2. Implementing incentive-based campaign messaging, which showed early
success but now requires new targeting strategies for uncontrolled patients.
