Applying QI Approaches for Better Cholesterol Management

  • The video begins with the basics of disease management and risk factor identification for the most prevalent chronic diseases, then focuses more specifically on the importance of cholesterol management. Resources from CDC and Million Hearts Programs are reviewed and shared with listeners so they in turn can use these materials with their care teams and patients. Common QI performance metrics are defined and discussed including strategies to integrate these metrics into everyday practice.

Evaluation Planning Part 1: Measuring with Purpose and Alignment to Achieve Impact

  • This series of trainings provides technical assistance related to basic practices and key principals for effective outcomes evaluation. The goal is to support Million Hearts grantees with collecting the right data to effectively demonstrate impact, inform ongoing quality improvement, and build on their success. The series provides concrete strategies and tools to facilitate effective data collection and analysis, determine performance metrics, and operationalize evaluation using standard methodologies. Part 1 of the series begins with the purpose of measurement and evaluation, the intersection of improvement and impact, and developing a roadmap for change with consideration of various stakeholders.

    Clinical Improvement Plan

    • The clinical improvement plan was introduced in the July Million Hearts Learning Collaborative meeting. The CIP should align with both the work plan and evaluation plan, but is not a check-in on all tasks. The focus is on selecting high-impact activities that would benefit from technical assistance, frequent monitoring and process improvement. Practices should select 1-3 activities at a time to focus on, and activities may change over time. Practices should develop a draft CIP in September 2021, and share updates monthly during the Million Hearts grantee meetings.
