  • Framing the Topic in Data: Defining a High-Risk Cohort through the Men’s Health Initiative
    During this session, DC Health will present the data used to develop and inform the Men’s Health Initiative,
    how the three components DC Health is implementing to support this population. DC Health will also explain how
    this defined cohort should be included in Grantee Workplans, and how data will be leveraged to identify trends in the population.

  • Selecting a High-Risk Cohort: Best Practices
    During this session, HMA will discuss considerations for determining a high-risk cohort. Grantees will have an opportunity to describe how they have determined their project cohorts.

  • Tools for Identifying a High-Risk Cohort
    During this session, DCPCA will discuss data tools available to member health centers to use data to identify high-risk hypertension, diabetes, and high cholesterol cohorts, including tools within their eCW toolkit.
