This presentation will cover trends in Medication use among DC residents at a population level and trends in condition control among patients seen at grantee sites. DC health will further dive into population groups to assess health disparities in medication use and condition control among DC residents.
2:20pm PopHealth Analytics: Medication Adherence Reports

This session will provide an overview of the Health Information Exchange (HIE) and the Pop Health Analytics tool. It will also explain the medication adherence prediction model- what data it uses, how the algorithm identifies high‑risk cohorts, and how providers can use the reports to identify patients at risk for non‑adherence. A demo of the hypertension and statin reports will be included.
