District-wide Heart Disease and Stroke Prevention & Innovative Heart Health Learning Collaboratives

Heart Disease and Stroke Prevention Learning Collaborative sessions are typically held on the third Wednesday of each month at 2:00 PM. Hosted by the Chronic Disease Division at DC Health, these sessions bring together experts from diverse sectors to build consensus and develop strategies aimed at improving heart health outcomes across the District

    This presentation will cover trends in Medication use among DC residents at a population level and trends in condition control among patients seen at grantee sites. DC health will further dive into population groups to assess  health disparities in medication use and condition control among DC  residents.  

2:20pm PopHealth Analytics: Medication Adherence Reports  

  • Corrine Jimenez & Vinay Balani, CRISP DC  
    This session will provide an overview of the Health Information Exchange (HIE) and the Pop Health Analytics tool. It will also explain the medication adherence prediction model- what data it uses, how the algorithm identifies high‑risk cohorts, and how providers can use the reports to identify patients at risk for non‑adherence. A demo of the hypertension and statin reports will  be included.
