- Maximizing Your Care TeamThis session will address strategies for maximizing care teams and leveraging resources beyond the clinic walls. HMA will highlight learning collaborative participants’ historical care team composition and EHR supports and lead a discussion of how team composition has changed and what participants are doing to maintain patient engagement during care-team turnover. This discussion will also feature participant polling to inform future programming.
