Million Hearts Learning Collaborative

• Grantee Feedback Update and Patient Engagement Strategies from Grantees

  This session provides an update and initial summary on Grantee Evaluation Survey results to inform future program planning. The session will also showcase two Million Hearts Grantees, Metro Health and Family and Medical Counseling Services, to share interventions, successes and challenges to support programming and patient engagement. Metro Health will discuss their interventions to support Million Hearts programming through partnerships with MCOs, identification of care gaps, and data reporting and analysis. Family and Medical Counseling Services will discuss interventions, successes and challenges to support patient engagement specific to diabetes and hypertension.
