This presentation will review how your organization can connect patients to Produce Plus, a farmers market incentive program for residents with or at risk for chronic conditions.

Building Healthy Habits – Tips and Tricks for Patients

The session will offer practical tools to help patients build and maintain healthy eating habits. The session will explore basic nutrition awareness, share tips for navigating the grocery store and highlight tactics that may help increase comfort in the kitchen. Nutrition educator, Brandin Bowden, MSc. will pull strategies from over a decade of work designing community-based interventions that have help individuals transform their relationship with diet and food.

Enhancing Patient Self Management: Motivational Interviewing Series

Motivational Interviewing sessions are the last in our series on Enhancing Patient Self-Management supporting practice efforts to enhance the care and support patients need to optimize their health. We present Motivational interviewing in 6 short segments. The first segment provides an overview of motivational interviewing and sets the stage for specific tools and strategies for putting each tool into practice. The second addresses open-ended questions and strategies for putting them into practice. The third discusses change talk and strategies for understanding and encouraging change talk. The fourth focuses on reflective listening and strategies to provide neutral and accurate restatement of the individual’s content and feelings in order to encourage elicit change talk. The fifth addresses how to understand and respond to statements we might perceive as resistance or sustain talk and strategies for encouraging change talk. The last session is on building conviction to change. A practice guide supports the development of skills through various exercises and practice.
